Yes, it's better to tell people this is a survey/interview,
rather than just tell us this is your homework.
However, if this is an interview not a survey,
your teacher probably wants you to practice speaking,
not reading or writing.
So I cannot help you in this way, sorry.
BTW, my English is not so good,
but there are obvious grammar errors in your article.
You might want to correct them.
