Copies should be single-sided; do not combine multiple family members on one page. 2) White I-94 Card: If in the U.S., current copies of Forms I-94 (front and back) for you and all family members, making sure that the admission date-stamp is legible on all copies. Please do not include more than one I-94 per page. Copies should be single-sided and made on full-size 8.5" x 11" paper. 3) Form I-20: If you are in, or you previously held, F-1 status, provide complete copies of all pages of Forms I-20 for you and any family members. 4) Form DS-2019: If you are in, or you previously held, J-1 status, provide complete copies of all pages of Forms DS-2019 for you and any family members. 5) Employment Authorization: Provide copies of any EAD cards issued to you or any family members while in E-2, F-1, F-2, J-1, J-2 or L-2 status. 6) Forms I-797, I-129S: If you or your family are currently holding, or previously held, any nonimmigrant status such as H, L, O or E status, provide copies of all Forms I-797 (approval notices) for you and family members. If entered on a Blanket L, provide copy of stamped Form I-129S. 7) Resume: Provide an up-to-date resume listing job duties by employer and not by project. 8) Educational Documents: Copies of all university-level and post-graduate educational documents (diplomas, transcripts, marksheets). Primary and secondary school documents are not needed. If your degree has not yet been issued, provide official transcripts and a letter from the university confirming that all degree requirements for the degree have been completed, including the submission and defense of thesis, if required, and that you will graduate on [date]. 9) Letters of Experience: If you have not completed a bachelor’s degree or higher program, you will need to provide a letter(s) from previous employers confirming your dates of employment, job title, and detailed job duties. Generally, three years of professional experience is equivalent to one year of university. If you are not certain whether or not you need to provide letters of experience, please contact your designated FRAGOMEN professional to discuss. 10) Court Records: Provide copies of any records regarding any arrests, citations, charges, indictments, fines or imprisonment for breaking or violating any laws or any ordinances, excluding traffic violations, for you and/or any family members. 11) Legal evidence of name change except by marriage (if applicable) If your spouse and/or your children under the age of 21 are applying for derivative H-4 status: 1) Marriage Certificate: A copy of your official marriage certificate. 2) Birth Certificates: A copy of each child’s registered birth certificate. The birth certificate must state the date and place of birth and full names of both parents, including the maiden name of the mother.
