我括起來的那個句子是形容詞子句 修飾the person
原本應該是這樣
To order the catalog, simply include the name and address of the person ("whom"
the catalog should be sent "to") on the attached form.
可以把to提到關代(whom)前面 於是就變成了to whom
翻譯大概是這樣
"為了訂閱目錄，目錄應送達的那些訂閱者僅需在附表上留下姓名以及地址"
