0. 我不是針灸專業，只是看大家吵，忍不住隨便找點針灸跟中風的EBM文章，我也不知道針灸可以做到甚麼程度，因為這涉及個人醫術是否想要一再精進，有沒有適當資源環境配合，臨床經驗，研究.... 1. 我沒時間讀查到的論文，只有看標題跟abstract，想文章細讀，有興趣的人自己念。想吵不要只是在自己"過去印象"上吵，至少在這些論文的基礎上去吵，吵著吵著我們還可以一再進步，成長。不過如果，我不用花太多時間就可以在pubmed查到，為什麼我們都在等別人查給你？ BMC Complementary and Alternative Medicine (2016) 16:361 Efficacy and safety assessment of acupuncture and nimodipine to treat mild cognitive impairment after cerebral infarction: a randomized controlled trial J Am Med Dir Assoc. 2016 Dec 1;17(12):1114-1122. BMC Complementary and Alternative Medicine (2016) 16:226 Additional effects of acupuncture on early comprehensive rehabilitation in patients with mild to moderate acute ischemic stroke: a multicenter randomized controlled trial 2. 既然要談EBM所以選至少RCT，如果有興趣看acute stroke的大鼠機制，也有一篇，但動物實驗在EBM證據等級有點低，但想抓人來切腦，IRB不會過吧...勉強看看囉 BMC Complementary and Alternative Medicine (2017) 17:470 The ameliorative effect of bloodletting puncture at hand twelve Jing-well points on cerebral edema induced by permanent middle cerebral ischemia via protecting the tight junctions of the blood-brain barrier 井穴放血這個概念，有人就拿來做動物實驗了...也發paper了... 3. 雖然這裡吵的是acute stroke 前面查到的論文與這個主題有差異，我也沒時間慢慢讀查acute stroke，不過，有篇文章其實有人設protocol想做 acute stroke RCT研究，有興趣自己看。不過只是protocol目前療效沒有答案。 BMC Complementary and Alternative Medicine (2017) 17:112 Acupuncture for acute moderate thalamic hemorrhage: randomized controlled trial study protocol 這個文章提醒我們的重點在，如果現在沒有EBM證據，為什麼不可以突破現境去研究？？ 4. 其實有些狀況沒辦法打t-PA，太慢送到醫院、醫療資源不足之地，如果可以團隊合作，配合針灸有時候不見得不是一條突破現在療效困境的方法，不滿足現有療效，持續改進自己知識、醫術、技能、結合團隊其他人的力量，最終讓療效提高，應該是一個醫師一生的追求。不要畫地自限，不管是畫自己，還是畫別人的地。如果最終我們都是想讓病人好，那為什麼要吵？而不了解彼此，尋求合作？ 5. 吵架有點浪費彼此時間，除非是完全不能溝通，吵一吵發洩怒氣自己爽一下也好理性討論-求同存異，才是最終能解決爭議的方法。最後最重要的是....真的想讀acupuncture for acute stroke 6. Cochrane Systematic review Acupuncture for acute stroke中有一大堆論文可以讓你好好讀。結論Authors' conclusions Acupuncture appeared to be safe but without clear evidence of benefit. The number of patients is too small to be certain whether acupuncture is effective for treatment of acute ischaemic or haemorrhagic stroke.
