學究一下，摘錄 ACLS provider course 裡面 adenosine 的適應症
1. First drug for most forms of stable narrow-complex PSVT.
Effective in terminating those due to reentry involving
AV node or sinus node.
2. May consider for unstable narrow-complex reentry tachycardia
while preparations made for cardioversion.
3. Wide-complex regular tachycardia, thought to be or previously
defined to be, reentry SVT.
4. Does not convert atrial fibrillation, atrial flutter, or VT.
5. Undefined, stable narrow-complex SVT as a diagnostic maneuver.
原po提出的第一點，orthodromic AVRT，參照上面，問題不大 XD
第二點，WPW-AF，其實是 irregular 的 wide-complex tachycardia，
（理論上，atrial wave 可以從 AV node 或是 AP 往下傳，但是 AP 在
anterograde方向的 ERP 短，經過 AP 往下的佔大部分，而且心室的
response快到兩三百，加上 AF 造成的 aberrant conduction，況且
AP 可能不只一個，這些因素加起來，QRS 看起來都是寬的...）
在急性處理方面，adenosine/digoxin/class II & IV 是禁忌。
如果病患穩定，procainamide 或是 amiodarone 都可以試試，
（amiodarone 特別用在 LV dysfunction 或是 heart failure）
如果病患不穩定，或是藥物沒效，就選擇 DC cardioversion。
另外要 call cardiac EP experts stat!
另一個值得了解的是，怎麼用 adenosine 來鑑別穩定的 regular NCT，
